West Texas Centers is seeking a highly motivated and organized IDD Enrollment Coordinator to support individuals with intellectual and developmental disabilities (IDD) through enrollment, transition, and coordination of community-based services.
This position plays a vital role in ensuring that individuals moving from state facilities or nursing homes successfully transition into community living. The Coordinator will oversee enrollment into HCS, TxHmL, and State Supported Living Centers (SSLCs) programs, while also maintaining strong partnerships with local providers and state agencies.

Starting pay: $21.84
Serve as the LIDDA's HCS and TxHmL Enrollment Coordinator.
Develop and maintain relationships with public and private providers within the service area.
Coordinate enrollment and transition planning for individuals leaving nursing facilities or state facilities.
Complete or oversee assessments and documentation (ICAPs, ID/RCs, IPCs, PDPs).
Serve as the Enhanced Community Coordinator (ECC), ensuring compliance with state requirements and timely reporting.
Provide PASRR Evaluations, habilitation coordination, and clinical consultation as needed.
Serve as the LIDDA's GR CFC Program Coordinator, ensuring assessments and documentation are completed accurately and on time.
Participate in planning meetings, pre- and post-move monitoring, and service coordination activities.
Advocate for individuals served and ensure their rights, dignity, and preferences are respected.
Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred.
Experience working with individuals with intellectual and developmental disabilities (IDD) strongly preferred.
Ability to travel within the service area
Valid driver's license and reliable transportation required.
