Position Summary:
The Service Administrator Coordinator provides ongoing case management and coordination services to individuals with intellectual and developmental disabilities. This role assists individuals in accessing medical, social, vocational/educational, and other needed services, while advocating for their rights and supporting their long-term goals.
Key Responsibilities:
Develop and maintain long-term relationships with individuals and their families.
Facilitate and coordinate Service Planning/Interdisciplinary Team (IDT) meetings.
Coordinate and document the development of the individual's Person-Directed Plan (PDP) or Service Plan.
Monitor service implementation to ensure responsive and effective delivery.
Provide advocacy and support to individuals on assigned caseloads.
Authorize services and track progress across HCS, TxHmL, and General Revenue-funded programs.
Maintain HCS/TxHmL interest lists.
Serve as Permanency Planner, CRCG/ARD/SSLC liaison, and LIDDA's GR CFC program coordinator.
Complete extensive documentation and ensure compliance with agency policies.
Travel as needed; may require the use of a personal vehicle.
Qualifications:
Bachelor's degree in a human services-related field preferred.
Experience working with individuals with intellectual and developmental disabilities is highly desirable.
Strong organizational, communication, and documentation skills.
Ability to work independently, manage multiple priorities, and maintain professionalism.
Valid driver's license and reliable transportation required.
