783 Rural Jail Caseworker

The Case Manager provides on-going case management and rehabilitative services to individuals with behavioral health needs. This role is essential in helping clients gain access to medical, social, vocational, and educational services while supporting their recovery and independence within the community.

The position requires flexible hours, extensive documentation, and self-directed performance. Travel and the use of a personal vehicle are required. The Case Manager reports to the Regional Program Manager.

Salary:$21.84

Key Responsibilities

Assist individuals in accessing medical, social, vocational, and educational services

Develop individualized service and recovery plans

Coordinate, monitor, and document all services provided

Advocate for client needs and rights within the community

Provide crisis intervention and resolution

Deliver rehabilitative services focused on reducing symptoms and disabilities related to mental illness

Facilitate skills training and psychosocial rehabilitation to enhance daily functioning

Perform symptom management services, including monitoring and wellness checks

Maintain regular contact with clients, families, and service providers

Ensure compliance with agency policies, HHSC standards, and all regulatory requirements

Position Requirements:

  • Required: Bachelor's or advanced degree from an accredited college or university
  • Must have and maintain a Valid Drivers License and driving record that is acceptable for insurability through our insurance carrier.
  • Must pass background check and drug screening.
  • Applicants must reside within 70 mile radius of duty site
