Jail Transition Caseworker

West Texas Centers is searching for a highly motivated individual to be part of the team as a

Jail Transition Specialist. This position will work closely with jail staff, law enforcement and

many other community stakeholders to help individuals with mental health challenges

transition back into the community from the jail setting. Comprehensive benefit package

assistance program, etc.  Salary Range: $21.84-$24.93

Job Functions include assisting individuals in gaining needed services:

 Creating a transition plan for each individual assigned to caseload

ensuring required services are offered and/or provided

 coordination of services

service monitoring

advocacy

 crisis resolution

engaging with clients and other community members to complete transition plan

Requires flexible hours, involves extensive paperwork, and will require self-directed

performance. Extensive travel is required daily. Personal vehicle may be required with mileage

reimbursement.

Position Requirements:

Required: Bachelor's or advanced degree from an accredited college or university. 15

hours of social/human behavioral services

 Must have and maintain a Valid Drivers License and driving record that is acceptable for

insurability through our insurance carrier. 

 Must pass background check and drug screening.
