The Case Manager provides on-going case management and rehabilitative services to individuals with behavioral health needs. This role is essential in helping clients gain access to medical, social, vocational, and educational services while supporting their recovery and independence within the community.
The position requires flexible hours, extensive documentation, and self-directed performance. Travel and the use of a personal vehicle are required. The Case Manager reports to the Regional Program Manager.

Key Responsibilities:
Assist individuals in accessing medical, social, vocational, and educational services
Develop individualized service and recovery plans
Coordinate, monitor, and document all services provided
Advocate for client needs and rights within the community
Provide crisis intervention and resolution
Deliver rehabilitative services focused on reducing symptoms and disabilities related to mental illness
Facilitate skills training and psychosocial rehabilitation to enhance daily functioning
Perform symptom management services, including monitoring and wellness checks
Maintain regular contact with clients, families, and service providers
Ensure compliance with agency policies, HHSC standards, and all regulatory requirements
Will provide support services to individuals with Intellectually Developmentally Disabled individuals. Services will be provided within the community and in the person's home. Services include Employment assistance, Individualized Competitive Employment/Supported Employment, Community Supports/Supported Home Living/PAS HAB, and In-Home Respite. Responsibilities include providing training, supervision, and/or support in the areas of self-help, independent living, community participation and involvement, social and peer development, employment, financial management and health maintenance. Required to work flexible hours and may be required to use personal vehicle to perform job duties. Will be supervised by Support Services Manager.

Job Qualifications
Position requires either a High School Diploma, GED, or 3 letters of reference and the completion of a WTC competency test. Must be proficient in reading and writing.

Asset Management Specialist
General Purpose:
The Asset Management Specialist is responsible for maintaining the WTC warehouse, delivering supplies, ensuring routine maintenance and inspection of agency vehicles and facilities. This position also supports operational logistics and performs minor building repairs. Work is performed under the supervision of designated management and requires adherence to safety, compliance, and confidentiality standards. A personal auto is required as some travel is required. High school education or GED required. Pay range $13.34 - $15.22/hour

The Occupational Therapist is responsible for assisting families in gaining access to needed services and supports as outlined in the Individualized Family Service Plan (IFSP). The Regional Crisis Worker will be responsible for the daytime crisis coverage for assigned counties from 8 to 5, five days each week. On-call after-hours coverage is essential. The Regional Crisis Worker will serve as crisis back-up if needed, will provide crisis support services for clients in Service Package 0 for assigned counties, and will provide services to post SMHF discharged clients for up to 30 days if needed. The Regional Crisis Worker is responsible for providing services to clients as directed by the program manager to assist case managers in delivering all prescribed rehab services. Falsification of any document constitutes grounds for dismissal. The Regional Crisis Worker will complete paperwork and data entry on the computer system in a timely manner. Requires QMHP credential, computer and writing skills, self-directed performance and the ability to work with mentally ill clients, families and agencies in a professional, caring manner. Must have a valid Texas Driver's license and a driving record that is acceptable for insurability through the Center's insurance carrier. This position is supervised by the Regional Program Manager.
