Case Manager HB13

Provides Triage/Liaison between the Nolan County Jail, West Texas Centers and community resources to assure continuity of care for persons identified as having a reported or potential mental health or substance abuse disorder. Duties include coordinating behavioral and physical health needs upon release from jail; Provides behavioral health consultation and follow up with primary care and behavioral health care patients. Performs case management activities including, service linkage; providing behavioral health assessments documentation; coordination of clinic services; monitoring; advocacy; crisis resolution; liaison services; and other support services. Duties of the position will require frequent interactions with jail staff, community members, Primary Care Physicians, hospital and WTC program staff. Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Bachelor degree from an accredited college or university with a major in social, behavioral, or human services. Must have a valid Texas driver's license and a driving record that is acceptable for insurability through the Center's insurance carrier. Will participate in community coalition meetings, trainings and Criminal justice training seminars as designated.  Supervised by the Justice Involved Program Manager and West Texas Centers, Deputy Director of Behavioral Health of Essential Services.
