Position Summary

The LPHA provides diagnostic assessments, eligibility determinations, counseling

services, crisis consultation, and clinical support services to children, adolescents, and

adults experiencing mental health concerns.

Depending on assignment, the position may focus on:

? intake and assessment services,

? counseling services, or

? a combination of both.

Clinicians work onsite from their assigned clinic and may remotely connect to other

clinic locations to provide behavioral health services through telehealth platforms.

Responsibilities

? Complete diagnostic assessments and eligibility determinations

? Develop treatment recommendations

? Provide individual, family, and group counseling

? Deliver evidence-based interventions, including CBT

? Complete timely clinical documentation in the EHR

? Participate in interdisciplinary treatment planning

? Provide consultation and support to clinical staff

? Participate in crisis consultation rotation

Qualifications

? Unrestricted Texas license as LPC, LCSW, LMFT, or Licensed Psychologist

? Strong clinical assessment and documentation skills

? Ability to work independently and collaboratively

The Case Manager provides on-going case management and rehabilitative services to individuals with behavioral health needs. This role is essential in helping clients gain access to medical, social, vocational, and educational services while supporting their recovery and independence within the community.

The position requires flexible hours, extensive documentation, and self-directed performance. Travel and the use of a personal vehicle are required. The Case Manager reports to the Regional Program Manager.

Advocate for client needs and rights within the community

Provide crisis intervention and resolution

Perform symptom management services, including monitoring and wellness checks

Maintain regular contact with clients, families, and service providers

Ensure compliance with agency policies, HHSC standards, and all regulatory requirements

• Position Requirements:
• Required: Bachelor's or advanced degree from an accredited college or university
• Must have and maintain a Valid Drivers License and driving record that is acceptable for insurability through our insurance carrier.
• Must pass background check and drug screening.
• Applicants must reside within 70 mile radius of duty site

Will provide support services to individuals with Intellectually Developmentally Disabled individuals. Services will be provided within the community and in the person's home. Services include Employment assistance, Individualized Competitive Employment/Supported Employment, Community Supports/Supported Home Living/PAS HAB, and In-Home Respite. Responsibilities include providing training, supervision, and/or support in the areas of self-help, independent living, community participation and involvement, social and peer development, employment, financial management and health maintenance. The Regional Crisis Worker will be responsible for the daytime crisis coverage for assigned counties from 8 to 5, five days each week. On-call after-hours coverage is essential. The Regional Crisis Worker will serve as crisis back-up if needed, will provide crisis support services for clients in Service Package 0 for assigned counties, and will provide services to post SMHF discharged clients for up to 30 days if needed. The Regional Crisis Worker is responsible for providing services to clients as directed by the program manager to assist case managers in delivering all prescribed rehab services. Falsification of any document constitutes grounds for dismissal. The Regional Crisis Worker will complete paperwork and data entry on the computer system in a timely manner. Requires QMHP credential, computer and writing skills, self-directed performance and the ability to work with mentally ill clients, families and agencies in a professional, caring manner. Must have a valid Texas Driver's license and a driving record that is acceptable for insurability through the Center's insurance carrier. This position is supervised by the Regional Program Manager.
