129 Hospital Transition Support Liaison

West Texas Centers is searching for highly motivated individuals to provide ongoing Hospital Transition support to the people we serve. The Hospital and Jail Liaison facilitates discharges and continuity of care for individuals who are consumers of West Texas Centers or are from our catchment area and are admitted to State Mental Health Facilities, contract private psychiatric facilities, or private facilities known to the Center.

This position works closely with hospitals, jails, psychiatrists, and internal staff to ensure timely coordination of services, aftercare, and psychiatric appointments.

Starting Pay:$21.84

Key Responsibilities:

Receive and process discharge paperwork from State Mental Health Facilities and private psychiatric facilities

Check fax/email daily for discharge notifications and upload documentation into client charts

Schedule aftercare assessments with QMHP staff and notify appropriate teams (including MCOT)

Assist clients with trial placements and coordinate with service coordinators and SMHF staff

Track and enter SMHF bed day utilization and alternative placements

Schedule 46B evaluations with WTC's Forensic Psychiatrist

Maintain continuous communication with contracted county jails in the catchment area

Receive authorization and schedule psychiatric appointments for jail-based services

Schedule jail psychiatric services within required timeframes

Communicate appointment details back to jail staff and document activity in "Jail Tracker"

Assist with weekly High-Risk meetings and documentation

Send weekly High-Risk reports to designated staff

Maintain safe and insurable driving status and vehicle

Perform other duties as assigned


Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Travel and the use of a personal vehicle is required.

  • Required: Bachelor's or advanced degree from an accredited college or university
  • Must have and maintain a Valid Drivers License and driving record that is acceptable for insurability through our insurance carrier.
  • Must pass background check and drug screening.
  • Applicants must reside within 70 mile radius of duty site
