Position Summary:
The Service Administrator Coordinator provides ongoing case management and coordination services to individuals with intellectual and developmental disabilities. This role assists individuals in accessing medical, social, vocational/educational, and other needed services, while advocating for their rights and supporting their long-term goals.

Key Responsibilities:

  • Develop and maintain long-term relationships with individuals and their families.
  • Facilitate and coordinate Service Planning/Interdisciplinary Team (IDT) meetings.
  • Coordinate and document the development of the individual’s Person-Directed Plan (PDP) or Service Plan.
  • Monitor service implementation to ensure responsive and effective delivery.
